Precise Personalized Care Hospice was started in 2017 by three Doctors inspired to set a new standard of hospice care after years of failed expectations by other hospice companies. Dr. Mohammed Amir, Dr. Sultan Chowdhary and Dr. Najam Khan created Precise Hospice to give personalized care to each patient during their journey. Precise Hospice strives to give patients, families, physicians and facility staff individualized care plans and communication to ensure continuity of care between all involved.
